PROJECT INFORMATION
-The template is available in Microsoft Word format by clicking here. If you do
not have Microsoft Word on you computer please email at createacredit@selfhelpinc.org
and let us know what programs you do have that we may be able to transfer the file
into.
- Save the template to your computer.
-Make sure to keep a blank copy of this template on your computer, so you may use
it over and over.
-When you fill in the template on your computer make sure to hit SAVE AS and re-name
it in the following way: yourpartnership-yourinitials-nameofbook (example: hock-ld-oldmacdonald.doc)
-When all of this is completed, either email the finished project to createacredit@selfhelpinc.org
or postal mail to the address below, making sure you use the correct Partnership
connection that you are in:
-Once it is received, CPC will review your input and issue you a Certificate of Participation,
within 1 month. The certificate will include the CPC title, date, hours (2), name
of project and description of the activity and signature of a CPC staff member.
-If for some reason you do not receive your Certificate of Participation, please
don't hesitate to call.
-EEC has agreed that we can award you 2 training hours for this project and will
accept no more than 10 hours total on this single topic for each fiscal year (July-June).
-Please make sure all projects are in for that year by June 1, this will allow CPC
staff to review them, finalize them and send out certificates before the summer begins.
-Once all submissions are in and finalized we will create a CD-Rom, which will have
everyone's book projects on it, for all participants to have as a resource to use
forever.
