Summary:
To provide wraparound services to help children and families develop the skills they need to foster change and growth, and reduce the instance of out-of-home placement. Family Based Services are innovative community-based providing family support to strengthen families in crisis or facing major challenges.
Responsibilities:
- Assess individual child/youth needs, and develop safe and helping relationships.
- Develop a positive working relationship with parents to provide on-going support and services.
- Establish and maintain positive working relationships with DSS and other community agencies providing services to the families.
- Participate in the development of Treatment Plans.
- Provide ongoing supports related to achievement of Treatment Plan goals and objectives.
- Provide direct case management services for families and children.
- Provide advocacy for children and families as needed.
- Provide supportive services to foster/biological families, as well as identified children/youth.
- Assist with transportation as needed.
- Collaborate with internal and external resources and community providers to ensure timely, safe and appropriate placement and services.
- Participate in all team meetings as appropriate, i.e. Individual Education Plans, treatment plan meetings.
- Participate in staff meetings, group and individual supervision and case reviews.
- Comply with all Self Help, Inc. and DSS documentation requirements.
- Maintain compliance with all Self Help, Inc. policies and procedures, DMR regulations and Family Support guidelines including maintaining progress notes and completion of monthly reports.
- Perform other job-related responsibilities as assigned.
Qualifications:
Bilingual in English and one (or more) of the following languages: Cape Verdean Creole, Spanish or Haitian Creole; Bachelor’s degree in related field; valid driver’s license and vehicle; 1+ year case management experience; willingness and capacity to be mobile in the community; commitment to Family System Focus; Computer literate.
Available and Application:
One full-time, 35 hours per week and one part-time, 20 hours per week position. Self Help, Inc.
